Secondary erythrocytosis versus true polycythemia

Polycythemia vera is a different animal. It is a primary bone-marrow disorder driven by a JAK2 mutation, in which the marrow overproduces red cells on its own regardless of signals — and, tellingly, EPO is usually low rather than high. The opposite problem is worth naming because testosterone users create it themselves. A hematocrit that is too low — from over-aggressive blood donation or phlebotomy stripping your iron — brings its own fatigue, breathlessness on exertion, and poor recovery. How to lower a high hematocrit

Treat phlebotomy as a shared decision with your doctor, not an automatic monthly ritual. For most men, adjusting dose and injection frequency is the safer first move, and phlebotomy is reserved for genuinely high, symptomatic hematocrit — ideally without letting ferritin crash in the process.
